-------------------------DOSAGE AND ADMINISTRATION--------------------------­ • All patients should be evaluated for bleeding disorder risk before prophylactic administration (2.1) • The recommended dose is 15 mg every 12 hours administered by subcutaneous injection given up to 5 to 15 minutes prior to surgery (2.1) • Administer after induction of regional block anesthesia, if used (2.1) • Reduce dose in patients with renal impairment (2.2).
